Opinion: The Hidden Cost of Perfection

We have never had more information about how to live well. But when every choice becomes something to optimise, wellbeing itself can start to feel like another form of pressure.

Knowing More Is Only Part of the Problem

There is always another metric to improve, another habit to build, another supplement to consider, another routine to perfect. Much of this advice is well-intentioned. Some of it is genuinely useful. But the question worth asking is what happens when caring for ourselves starts to feel like another performance.

My professional background is in nutrition, public health and workplace wellbeing, and one recurring observation is that information alone rarely changes behaviour. Most people already understand the broad principles: sleep matters, regular movement is beneficial, and a varied diet rich in nutritious foods supports health.

Health psychology has long described an intention-behaviour gap – the distance between what people intend to do and what they actually do. A 2023 systematic review and meta-analysis of physical activity found that almost half of those who intended to be physically active failed to translate that intention into behaviour. The authors concluded that intention is necessary, but insufficient, for many people.

Health decisions are made inside ordinary life: while we are tired, stressed, caring for other people, answering emails, and managing deadlines. Environment, time and social context shape behaviour as much as motivation does.

Modern wellness culture often turns health into an individual optimisation project. Poor sleep becomes a sleep-hygiene project. Stress becomes a meditation target. Lack of exercise becomes a question of motivation. The message can gradually become uncomfortable: better health is available to anyone willing to try hard enough.

When Self-Care Becomes Self-Surveillance

Technology has created extraordinary opportunities to understand our health. A systematic review of 67 empirical studies on self-tracking found that tracking can support health and wellbeing, while also pointing to potential psychosocial downsides. Research into mobile health apps has similarly found that goal-setting, self-monitoring and feedback can support engagement with behaviour-change interventions.

A sleep score can become something to worry about. A daily step count can turn into a target that produces guilt when missed. A calorie target can become a boundary that feels dangerous to cross. A wearable can announce poor recovery before we have checked in with how we actually feel.

Researchers studying the quantified-self movement have documented both increased awareness and motivation and anxiety around interpreting personal health data.

The Pursuit of Perfect Health

Perfectionism is also increasingly present in wellness culture, often under more appealing labels: consistency, discipline, high performance, optimal health and clean living. These ideas can support healthy routines, but they can also turn health into a standard against which a person continually judges themselves.

A review and meta-analysis of adults found significant associations between perfectionism and symptoms of depression, anxiety and obsessive-compulsive disorder. Research has also begun to examine the relationship between perfectionism and orthorexia – an excessive preoccupation with eating in a way perceived to be healthy. A 2024 systematic review and meta-analysis found positive associations between dimensions of perfectionism and orthorexia. Orthorexia remains a developing area of research and is currently outside formal psychiatric diagnostic systems.

But food offers a clear example. Nutritious choices can support wellbeing, yet the same attention can gradually turn meals into a series of calculations: Is this clean enough? Does it fit my macros? Have I had enough protein? Should I avoid this ingredient? The focus shifts quietly from nourishment towards control.

The Algorithm Is Part of the Environment

Our ideas about wellness are increasingly shaped by algorithms as well as health professionals and public-health organisations. Every day we encounter idealised bodies, immaculate kitchens, elaborate morning routines, supplements, fitness regimes and increasingly sophisticated health advice. The quality ranges from evidence-based guidance to marketing, personal anecdote and fantasy presented as routine.

A 2026 systematic review examining social media and orthorexia found that diet- and fitness-related content and social-media exposure may contribute to preoccupation with healthy eating, while stressing that the evidence remains limited. Another recent review found links between exposure to idealised body images and fitness lifestyles and symptoms associated with orthorexia and muscle dysmorphia, alongside substantial gaps in the evidence.

Health becomes highly visible, comparable and continuously evaluated. Instead of relying mainly on internal cues – energy, hunger, fatigue, mood – we are repeatedly shown someone else’s version of what ‘well’ should look like. Comparison becomes part of the wellness environment.

When Optimisation Starts to Crowd Out Wellbeing

Modern wellness contains a paradox. Our tools for living better have become more sophisticated, while the number of things we are expected to manage has multiplied. We track sleep while struggling to switch off, monitor steps while spending hours in meetings, calculate the nutritional content of lunch while answering emails, and meditate before returning to an environment that keeps the nervous system under pressure.

The workplace makes this especially visible. An organisation can offer meditation apps, fitness challenges and healthy snacks, yet a culture built around constant availability, excessive workloads and skipped breaks will undermine much of that effort.

A More Human Definition of Wellness

A healthier relationship with wellness may begin with simpler questions. What would help me feel more rested? What food leaves me nourished and satisfied most of the time? Where can movement fit naturally into a day spent largely sitting? These questions sound less ambitious than optimisation, but they are often closer to daily life.

Self-compassion offers one alternative to constant self-improvement. A meta-analysis of 94 peer-reviewed articles involving almost 30,000 participants found positive associations between self-compassion, physical health and health-promoting behaviour, as well as evidence that multi-session self-compassion interventions could improve both. A separate systematic review and meta-analysis found that greater self-compassion was associated with lower eating pathology and fewer body-image concerns.

Self-compassion helps us stay healthy without being too strict with ourselves. It allows for imperfect days without making us feel that we have failed.

Wellbeing may be better judged by how comfortably healthy habits fit into the life we actually have. A simple meal with friends can support health in ways a tracking app will never capture. Choosing sleep over an unnecessary workout can be sensible recovery. Turning off a wearable when its numbers create anxiety can restore attention to the body’s own signals.

Sometimes progress means adding a habit. At other times, it means removing a demand: measuring less, eating with more ease, walking for pleasure, or resting without turning recovery into another task.

The next evolution of wellness may not be more optimisation, more data or more discipline. It may be a return to something simpler: habits that support health without requiring constant self-monitoring, and a definition of wellbeing that includes pleasure, flexibility and ordinary life.
